New Patient Registration
If you wish to register as a patient with the Leatside Surgery, please contact reception where you will be given the appropriate information and application forms. You will be required to present two forms of identification, one of which must be photographic. Patients register with the practice but may request to see any of the doctors. You do not have to see the named doctor, however, to help us to help you, we will identify one of our doctors as your ’usual doctor’ who will be the person mainly responsible for issuing repeat prescriptions.
If you live within our Practice Area then you will be able to register with the practice. If you live outside the Practice Area you may still be able to register with us as part of our Out of Area patient scheme. If you live outside of the boundary please ask at reception to see whether you are able to register in this way.

We ask that you give us 2 working days to get you registered with the practice before needing an appointment - however should you need urgent treatment we will endeavour to get you seen
Medical treatment is available from the date of registration. Please contact reception for further information. If you have any specific communication needs that we need to be aware of please inform reception.
Change of Address
If you change your address, or telephone number, please inform the surgery as soon as possible. This helps to keep our records up to date and ensures appointments are sent to the correct address.
Please also remember to inform the appropriate hospital department if you are awaiting an appointment or undergoing treatment.
If you move outside the practice area it is important that you register with another doctor as soon as possible to avoid complications, should you need to see a doctor urgently. The doctors are responsible for patients at the address shown on their medical records held at the practice. Should you wish to stay with the Practice you may be able to do so as an Out of Area Registration. Please ask at reception for details as to whether you will be able to continue as a patient under this scheme.
