Thank you. Your choice has been saved. You can change your preferences at any time.
If you wish to register as a patient with the Leatside Surgery, please contact reception by telephone or e-mail (firstname.lastname@example.org). You will then be sent the appropriate information and application forms. We will need two forms of identification, one of which must be photographic. Patients register with the practice but may request to see any of the doctors. You do not have to see the named doctor, however, to help us to help you, we will identify one of our doctors as your ’usual doctor’ who will be the person mainly responsible for issuing repeat prescriptions.
If you live within our Practice Area then you will be able to register with the practice. If you live outside the Practice Area you may still be able to register with us as part of our Out of Area patient scheme. If you live outside of the boundary please ask at reception to see whether you are able to register in this way. Children under the age of 16 are not able to be registered on the Out of Area registration scheme.
Our Practice Area can be viewed here
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information. If you have any specific communication needs that we need to be aware of please inform reception.
